How do you enroll or learn more about HFW?
HFW is an expanding program and is not available in all counties. A list of Care Management Agencies offering HFW can be found using the link below.
If you are interested in learning more and finding out if your family may be eligible, please contact the Care Management Agency offering HFW in your county. If you are currently enrolled in the Health Home Serving Children (HHSC) program, you can also speak with your care manager. While your current agency may not offer HFW, they can assist you in transferring to an agency that does if you are interested and eligible.
