The NYS Youth and Families ACHIEVE (Advancing Care through Health Integration and Evidence-based Effort) was a four-year project, funded through a Substance Abuse and Mental Health Services Administration (SAMHSA) System of Care (SOC) Expansion grant. The Office of Mental Health (OMH) was awarded the grant in October 2016.
Grant Focus:
- strengthening the statewide use of the SOC framework; and,
- implementing New York’s model of High Fidelity Wraparound (HFW) within Health Homes Serving Children (HHSC) for 12-21 year-olds with serious emotional disturbances and their families.
