SOC Action Planning
The SOC Action Planning Workshop builds on the idea that we can better support children and families if we work together. This workshop brings together county leadership from various systems serving children and families to collaborate, problem solve, and strategize on emerging community needs.
High Fidelity Wraparound
High Fidelity Wraparound (HFW) is a team-based planning process that provides coordinated, comprehensive, and youth and family-driven care to meet the complex needs of children, youth and families who have multiple systems involvement and who may experience serious mental health or behavioral challenges.
School Mental Health Quality Improvement Initiative
The SHAPE System offers schools and districts resources to support their school mental health quality improvement. Districts can use the tools available through SHAPE to compare their current school mental health systems with the standards laid out by the National Center for School Mental Health.
Family and Youth Peer Support
Family members and young adults have been important partners in the children’s systems as the SOC framework progressed. The NYS ACHIEVE initiative showed that using Peer Services is an important component of High Fidelity Wraparound (HFW) practice. As a result, the New York State’s SOC pilot continues to support the development of the Family Peer and Youth Peer workforce capacity.
