Systems of Care (SOC)

Post-Action Planning Workshop Implementation Toolkit	This toolkit was developed by the New York State (NYS) Office of Mental Health (OMH) to support counties in translating the energy, ideas, and momentum from their System of Care (SOC) Action Planning Workshop into meaningful and sustained progress. The SOC Action Planning Workshop is a facilitated, cross-system process that helps counties assess alignment with the national SOC framework, identify strengths, gaps, and priorities, and develop an initial action plan to strengthen collaboration on behalf of children, youth, and families.

Youth Resilience Toolkit	As a required component of the Project AWARE grant, the Youth Resilience Toolkit (YRT) was developed to share the range of resources and technical assistance that support this mission. The Toolkit (YRT) includes materials from state agency partners – the NYS Education Department and the NYS Office of Mental Health – as well as carefully selected external resources. These materials are intended to help schools build capacity to promote mental wellness among staff, students, and families, in close partnership with community collaborators dedicated to this shared mission.
