The goals of the Project include addressing barriers to coordinated care and expanding the project to reach more children and families throughout NYS.
We began this SOC expansion process with an initial three counties and now are focusing on two rural counties, Essex and Yates. By the end of the grant period, we expect that every county in NYS will be developing, or will have already established, a local SOC. We anticipate counties will also be using the HFW practice model.
