| Post-Action Planning Workshop Implementation Toolkit | This toolkit was developed by the New York State (NYS) Office of Mental Health (OMH) to support counties in translating the energy, ideas, and momentum from their System of Care (SOC) Action Planning Workshop into meaningful and sustained progress. The SOC Action Planning Workshop is a facilitated, cross-system process that helps counties assess alignment with the national SOC framework, identify strengths, gaps, and priorities, and develop an initial action plan to strengthen collaboration on behalf of children, youth, and families. |
